From the Richardson, Plano, and McKinney areas:

Take the 75 - Central Expressway south, Exit at I-635 west. Remain in the right lane when you enter 635, and exit on Hillcrest. Turn left (south) on Hillcrest, go under 635. The first traffic light after 635 is Hillcrest Plaza Drive. Turn right on Hillcrest Plaza Drive and follow the curve to the first building on the left, Concorde on the Creek. Suite 204 is on the main entrance level.

From DFW Airport, Downtown Dallas, and the North Dallas Tollway:

Take I-635 east. Exit at Hillcrest. Turn right (south) on Hillcrest. The first traffic light is Hillcrest Plaza Drive. Turn right on Hillcrest Plaza Drive and follow the curve to Concord on the Creek. Suite 204 is on the ground level.
